The Chronic Kidney Disease (CKD) Prevention Workstream of the South West Renal Network aims to improve outcomes for people living with CKD through an integrated and holistic approach to CKD care in the context of multi-morbidity. We aim to support integrated neighbourhood teams so that early CKD identification and treatment optimisation becomes the standard of care.
We are inviting your PCN to share information about CKD projects and activities. The purpose of this survey is to help improve CKD and wider cardiometabolic–kidney health by understanding what is working well across PCNs and where further support may be needed.
The survey takes approximately 10 minutes. Your input will help us:
Identify examples of good and innovative practice that could be shared across PCNs
Understand current challenges, gaps, and areas where support could have the greatest impact
Highlight key enablers of success such as workforce, tools, or pathways
Explore opportunities to strengthen kidney health within broader multi-morbidity and cardiometabolic pathways
The anticipated outcome is to build a clearer picture of effective approaches and improvement opportunities, enabling replication of successful models and targeted support for PCNs.
