In person event - Collaborating to Transform CKD Care Across the South West
Hosted by the CKD Prevention Workstream of South West Renal Network
This collaborative learning event brings together those involved in CKD care across the South West to:
- Introduce the CKD Prevention Workstream of South West Renal Network
- Share learning and provide peer support by showcasing the work that is happening in CKD care across the South West and nationally
- Explore innovative models of CKD care incorporating multi-morbidity and integrated neighbourhood working
- Understand what works well and share good practice
- Identify barriers and help people overcome them
