What is the purpose of the CKD Prevention Workstream?

    To improve outcomes for people living with CKD through an integrated and holistic approach to CKD care in the context of multi-morbidity, delivered by neighbourhood teams who understand their populations.

    What does the CKD Prevention Workstream aim to do?

    Aims

    • To support integrated neighbourhood teams so that early CKD identification and treatment optimisation becomes the standard of care.
    • To support the use of actionable data with structured support for primary care to adapt pathways and facilitate workforce development.
    • To promote integration between primary and secondary care and strengthen multi-disciplinary team working.
    • To promote equity in CKD care by supporting underserved areas and addressing barriers
    • To develop a community of practice to share learning and facilitate peer support.
    • To support ICBs in understanding, assessing, and improving the quality of CKD care.
    • To collaborate with ICBs to influence strategic commissioning decisions and secure sustained investment in CKD.

    What is the purpose of the CKD survey?

    To gather the below inform to inform the content of the face to face event including the identification of speakers at the event

    • To understand what CKD projects have been implemented in primary care in the South West.
    • To understand what CVRM and multi-morbidity projects have been implemented in primary care in the South West.
    • To understand how neighbourhood teams are involved in delivering CKD and CVRM care across the South West.

    What is the in-person CKD event?

    Hosted by the CKD Prevention Workstream of South West Renal Network 

    This collaborative learning event brings together those involved in CKD care across the South West to:

    • Introduce the CKD Prevention Workstream of South West Renal Network 
    • Share learning and provide peer support by showcasing the work that is happening in CKD care across the South West and nationally
    • Explore innovative models of CKD care incorporating multi-morbidity and integrated neighbourhood working
    • Understand what works well and share good practice
    • Identify barriers and help people overcome them
