  • This short survey aims to find out about your experiences of the outpatient follow-up service you received following your cancer diagnosis and treatment. 

    Your follow-up may have been referred to as personalised stratified follow up (PSFU), supported self-management or open-access follow up. 

    PSFU is a way of managing follow-up care if you have finished treatment for cancer. Instead of routine hospital appointments, you are supported to self-manage with:

    • Information about symptoms to watch for
    • Easy access to your care team if needed
    • Regular tests or scans (like PSA blood tests for prostate cancer)
    • Remote check-ins (by phone or email)
    • A personalised care and support plan (PCSP) to guide your recovery
    • Health and wellbeing information


    This survey is anonymous to enable you to share your honest feelings about your experience. 

    Please ensure you do not enter any personal identifiable details in the free text answers such as names of GP practices, hospitals, staff or any locations.
