Who should complete the survey?

    The survey is aimed at Chief Medical Officers (CMOs) or a nominated senior clinical leader with oversight of AKI performance and improvement within your Trust.

    Why are we asking you to complete this survey and how will responses be used?

    Your input will help build a clear picture of how AKI is currently detected and managed across the South West. The findings will:

    • Shape a personalised AKI performance report for your Trust.

    • Identify improvement opportunities and areas of impact.

    • Inform regional priorities and action plans for 2025/26.

    All responses will be collated and analysed at a regional level to highlight themes and variation. Individual Trust data will remain confidential and used only to inform your bespoke report and local improvement planning.

    How long will the survey take to complete?

    It takes around  5-10 minutes. The questions are concise and designed to capture a high-level view of current AKI practices, priorities, and challenges.

    What are the next steps following survey completion?

    Survey findings will feed into a personalised AKI report for your Trust. These reports will guide joint planning sessions and the development of targeted AKI improvement actions for 2025/26 and beyond.

    When will the personalised reports be available?

    Reports will be shared with each Trust in early 2026, ahead of collaborative workshops to co-develop improvement plans.
