Mental Health and Wellbeing Survey

NHS South Central West is working with Bracknell Forest Council and Slough Borough Council to understand what helps young people to look after their mental health and wellbeing. The Councils want to know what local support is already working well for young people and what else might be helpful for them to provide.

Giving your views in this survey will help to shape plans for future services.

This survey can be completed by a young person (if between the ages of 13 and 18), or by a parent/carer (for someone under the age of 18).

• Answers will be anonymous - this means nobody will know your name, age or where you live if you complete the survey.
• We ask that you do not share personal or medical information (if you do this in error it will not be used).
• If you don’t wish to answer a particular question you can simply skip to the next one.

It is important for you to know that this isn’t the place to get medical help or support, and these pages aren’t monitored 24/7.
