Frimley Community Mental Health Transformation - Professional survey

We invite you, as a professional who has been involved in developing, delivering or supporting mental health integrated community services (MHICS) and/or personality difficulties (PD) services; or a Mental Health Practitioner as part of the Additional Roles Reimbursement Scheme (ARRS); across East Berkshire and North East Hampshire, Farnham and Surrey Heath within the last year, to take a short survey about your experiences and the impact of the Community Mental Health Transformation programme in the Frimley Integrated Care System (ICS).

We want to hear from colleagues that directly support patients who access MHICS/SUN/MEP services (i.e., psychologists, mental health and wellbeing practitioners, community connectors, psychiatrists, pharmacists), those who refer into these services (including GPs, ARRS Mental Health Practitioners, and other professionals), and professionals involved in the daily management/ running of the services (i.e., Administrators, Service Managers)

If you decide to participate, you’ll answer questions about what went well and what could be improved. The results will help us create a report with recommendations to improve services, as part of an independent evaluation being carried out by NHS South, Central and West on behalf of Frimley ICS.

The survey is anonymous. We won’t collect any personal details, so your responses can’t be traced back to you. The survey takes roughly 10 minutes to complete.

Participation is voluntary. An answer is required to the first two questions to help us confirm your eligibility to take part in the survey. All other questions are optional, you can skip any you don’t want to answer or that you feel are not relevant to you.
