Frimley Community Mental Health Transformation - Service user survey

We have changed the way that we provide mental health services across East Berkshire and North East Hampshire, Farnham and Surrey Heath. This survey will help us understand your experience of care from our mental health integrated community services (MHICS) and/or personality difficulties (PD) services over the past year.

If you decide to take part, you’ll answer questions about what went well and what could be improved. The results will help us make recommendations to improve services, as part of an independent evaluation being carried out by NHS South, Central and West, on behalf of the local NHS.

The survey is anonymous, we won’t collect any personal details, so your responses can’t be traced back to you. The survey takes roughly 10 minutes to complete.

Participation is voluntary. An answer is required to the first two questions to help us confirm your eligibility to take part in the survey. All other questions are optional, and you can skip any you don’t want to answer. Your answers won’t affect the standards of care you receive.
