Bromsgrove and District PCN Health Inequalities

The health of people in and across Bromsgrove is varied compared with the England average with a mixed economy. Bromsgrove is one of the 20% least deprived districts/unitary authorities in England, however about 10.2% (1,605) children live in low income families and life expectancy is 8.9 years lower for men and 8.4 years lower for women in the most deprived areas of Bromsgrove than in the least deprived areas.

Within the Bromsgrove PCN there are 9 Practices with a list size of 77,000, sharing a border with Redditch and Birmingham impacting access and out of area care.

In the context of the changing landscape nationally there is an appetite to develop a long term PCN Health Inequalities Strategy that will set out a roadmap for key initiatives to tackle Health Inequalities across all 9 Practices. Building on the Core 20+ initiatives already underway, there is an opportunity to optimise the assets in Primary Care that will drive the delivery of population health management initiatives locally.

We are asking you to have your say, by contributing to the Ideas Wall below, we really want to hear about any thoughts or comments particularly in relation to the following;

  • What local initiatives have you developed to address health inequalities and what is working well and what isn't?
  • Where do you believe we should focus in the future so that we can continue to improve outcomes for people and address health inequalities in your communities?
  • What support and/or resources do you need to help you?
