South West H&J Medicines Optimisation
NHS England South West Health and Justice (H&J) Team have commissioned the Medicines Optimisation Team at NHS South, Central and West (NHS SCW) to support all secure sites across the South West region with Medicines Optimisation services.
As part of this support, we are undertaking an online survey to establish the service baseline for pharmacy provision across the region. The content of the survey is based on the national standards for optimising medicines and care for people in secure environments. Professional standards for optimising medicines for people in secure environments and Healthcare standards for children and young people in secure settings.
The results of the survey will be analysed and presented back to sites and commissioners to recognise areas of good practice and to provide further support on any areas which may require improvement.
How to complete the online survey:
- It is recommended that the survey is completed by an on-site pharmacist, clinical lead or lead pharmacy technician who has a thorough understanding of the pharmacy and medicines optimisation services on the site.
- Please submit one response per site only.
- Please do not include multiple sites per response.
- You can save your responses and come back to it later if needed. Only 'submit' once you are happy with your responses.
- A PDF copy of the survey is available on the main page of the questionnaire, also attached. This may be helpful if you wish to complete the survey with input from other relevant colleagues. Please note final submission must be on the online platform.
- Please submit the completed survey and attach any supporting documentation by 26th June 2024.
