Structured Medication Review Survey in Health and Justice Setting

The implementation and delivery of Structured Medication Reviews (SMRs) have been underway within Primary Care Networks (PCNs) across England since 2020. In alignment with the national priority to improve medicines optimisation and reduce polypharmacy, and to ensure patients in the H&J settings receive care equivalent to those in the wider community, NHS England Health and Justice (H&J) is undertaking a national programme to embed delivery of SMR in the H&J settings.

As part of this programme, we are asking all H&J adult prison/YOI sites across England to complete a short survey. The survey aims are to:

  1. Provide a comprehensive review of all prisons that have already implemented SMR service and how this looks across different prison types
  2. Gather information and identify areas of success
  3. Identify barriers and challenges of implementing SMRs
  4. Collate supportive and evidence based resources for training and development
  5. Provide tools to improve SMR roll out within H&J settings

We need one response for each prison so please nominate an appropriate person to complete the survey. In order to capture accurate information on the delivery of SMRs across all clinical services, the appropriate person completing the survey may need to liaise with other providers / team members who may deliver SMRs/ clinical care at the site, e.g., primary care clinicians, pharmacy staff, mental health and substance misuse teams.

How to complete the online survey:

  • Please submit one response per prison site only.
  • Please do not include multiple prison sites per response.
  • If you are answering for more than one site, you can use the same account (your email address) for each individual response.
  • You can save your responses and come back to it later if needed. Only 'submit' once you are happy with your responses.
  • A PDF copy of the survey is available on the right hand side of this page. This may be helpful if you wish to complete the survey with input from other relevant colleagues. Please note final submission must be on this online platform.
  • Please submit the completed survey and attach supporting documentation by 21st February 2024 *Date extended*
