Severnvale IPCT Project

If you can spare five minutes, we would be grateful if you could please complete this short survey relating to your recent experience in the practice. We are particularly interested in hearing from people who may have come to seek support for emotional wellbeing or mental health reasons, such as low mood, stress and anxiety. Please note, this survey is completely anonymous, and any information you share will only be used to improve our services. Thank you for your contribution.

This survey aims to understand staff experiences of the IPCT ways of working across Severnvale PCN, and how these experiences change over time as the IPCT matures. You may have already completed this survey in October 2022, but if so please do take the time to complete it again, with updated responses based on your current views.

If you have not completed this before, please still feel free to complete the survey as all staff responses are valid and insightful.

You will be asked to identify your role as part of the survey to support the evaluation analysis, however your information will be treated confidentially by the SCW Evaluation Team.
