Welcome and thank you for agreeing to participate in this short patient experience survey. The survey should not take any more than 10 minutes to complete. This survey is trying to find out about your experience of getting information about the treatment you received from the hospital. Participating in this study will help us to learn more about how we can continue to improve the information we give future patients. As this data collection is simply a survey any option you choose will have no effect on the care and treatments you may receive.
Here is a some information on what we are doing and why.
Cancer is personal. In Somerset we are committed to providing the best care possible when you have Cancer, giving you control and choice over the way your care is planned and delivered. This is part of a national initiative called Personalised Care and Support which aims to make sure that everyone living with cancer gets the right support to meet their needs.
We are currently asking for feedback on the information you are given when you complete a course of Cancer treatment, called an 'End of Treatment Summary'.
What is an End of Treatment Summary?
The aim of the treatment summary is to provide a summary of the treatment you have received; information on what to look out for in future including any 'red flags' or reasons to get back in touch quickly; details on how you can get back in touch with your specialist team including contact numbers; lifestyle advice and information on where to go for further advice if you need it.
A copy of this information is also sent to your GP to update them and alert them to any action they need to take to support your health now and in the future.
