Primary Care Networks
The scale of challenges currently faced in General Practice and across primary care networks is unprecedented and the increase in demand year on year is felt by all aspects of PCNs and those working in General Practice.
Strong General Practice and Primary Care services are essential if we are to have a high quality and responsive NHS, fit for the future. The resulting acceleration in development and delivery of new models of care, improved access and workforce strategy has meant that the Long Term plan and new GP contract are natural steps in the LLR journey towards Primary Care Networks and neighbourhood working supporting improved patient outcomes and an increasingly resilient General Practice.
Over the coming months we will use this platform to collaborate with each other and share ideas to help further develop our Primary Care Network model in which general practice can thrive.
We need to start sharing and talking more - this is tricky given our virtual working - but we hope that this space will help you to share some of the work that you are doing, explore challenges and co-produce solutions - to join the dots across our Networks and to encourage you to 'Join the Conversation'.
How to get involved
We want to involve as many PCN's to help build a picture of key challenges and as well as opportunities but also to determine how we can best support each other deliver the local ambition alongside that set out in the NHS Long Term Plan and NHS People Plan.
Through this platform, you can participate and share in the following ways:
Share your ideas - use the active ideas board to share your experiences and reflect on key successes and lessons you have learned throughout the pandemic response. You can also use this to articulate what you think are the greatest opportunities and challenges ahead for PCNs and share ideas on how we can address them. We will build on this content during the workshops, you do not need to register to see this, but you do need to register to leave a comment.
Share a case study- if you want to share any examples of work underway in your PCN that is helping to transform health and care then we would love to hear from you in this space. You can see the contributions others have made without signing in.
