We would like to consult with you to gather your expert insight and feedback on the proposed model for paediatric mental health liaison services at Frimley Park hospital.
The following survey has been designed to capture the views of professionals who work for services that support children and young people who may be admitted to Frimley Park Hospital in mental health crisis.
This survey will take approximately 30mins and will be used to inform service redesign.
